We do not store form submissions on our website's servers.
Our "Book a Session" and "Request Appointment" buttons link to our client portal, hosted by SimplePractice. SimplePractice is a HIPAA-compliant platform. Any information you enter there — including health history, insurance details, and payment information — is governed by SimplePractice's privacy policy and our practice's HIPAA Notice of Privacy Practices, not this document.
